Jackson Creek Fellowship Church takes photos and video during their sports camp and to use this media in church publications, website and social media. Photos and videos will NEVER be accompanied by captions or tags that include names or any other identifying information. Please indicate below if you wish to permit or not permit Jackson Creek to take photos and/or videos of your athlete.
Waiver
In consideration of my child’s participation in Jackson Creek Fellowship Church Sports Camp (dates: 6/22/26-6/26/26) at Jackson Creek Fellowship Church in Monee, IL, I, the undersigned parent/guardian, acknowledge that participation involves risk of injury. I hereby release, waive, discharge, and covenant not to sue Jackson Creek Fellowship Church, its pastors, staff, volunteers, agents, and affiliated organizations (collectively “Released Parties”) for any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) to person or property, whether caused by negligence or otherwise, that may be sustained by my child while participating in camp activities. I agree to indemnify and hold harmless the Released Parties from any claims, damages, or costs (including attorney fees) arising from my child’s actions. I certify that my child is physically able to participate and that I have provided accurate medical information on the camp health form. I understand this agreement is binding on my heirs and assigns.
